Perinatal Mental Health and Wellness Patient Satisfaction Survey

The Perinatal Mental Health Navigator pilot currently serves the South Shore. As we look to assess the effectiveness of this pilot, we are seeking feedback from patients who have received support from the Navigator. Your feedback will directly impact this program by providing us with a better understanding of the needs of the community we serve. This work will benefit future mental health support for pregnant and postpartum people.

Your responses are anonymous, and participation in the survey is voluntary.  

 The survey should take approximately 5-10 minutes to complete.

The navigator contacted me within the standard 7 day response time.

After I was referred to the perinatal mental health navigator, I had at least one conversation to set goals.

My needs were accurately assessed by the navigator.

I feel satisfied with the referrals made for me by the navigator.

When / if I needed a 'check-in' with the navigator, it was easy to arrange.

I felt like resources were individualized to my needs.

Overall, I feel that I have better support for my mental health struggles with help from the navigator.
