<p>The Community Heath Teams are part of the Nova Scotia Health in partnership with IWK Health. We offer free wellness programs and services in communities in the greater Halifax area. Our teams focus on helping people and families to make healthier lifestyle choices. Group topics include: Reducing Health Risks, Healthy Eating, Physical Activity, Mental Wellness and Parenting. Community Health Teams also support people and families to navigate community, government and health care systems. </p>
